2. Taxation Codes
Revenue codes linked with the non-covered Essure procedure have been removed from the table of revenue codes in Clinical Policy 1E-3, Sterilization Procedures, and revenue codes associated with sterilization procedures covered by the policy have been added.
3. Essure CPT Codes and Related Instructions
- Essure CPT code 58565 (Hysteroscopy, surgical; bilateral fallopian tube cannulation to induce occlusion by placement of permanent implants) will be withdrawn from Clinical Policy 1E-3, Sterilization Procedures, effective August 15, 2020. NC Medicaid no longer covers this code as of December 31, 2018.
- Clinical Policy 1E-3, Sterilization Procedures, will be updated on Aug. 15, 2020 to eliminate CPT code 58340 (catheterization and introduction of saline or contrast material for saline infusion sonohysterography (SIS) or hysterosalpingography). As of May 1, 2019, this code is no longer covered by NC Medicaid.
- Because the Essure sterilization treatment is no longer a covered service by NC Medicaid, the CPT codes 58579 (Unlisted hysteroscopy procedure, uterus) and 74740 (Hysterosalpingography, radiological supervision and interpretation) will be deleted from the policy as of Aug. 15, 2020. NC Medicaid will no longer cover CPT code 74740 as of August 1, 2020.
- The definition of hysteroscopic procedure, as well as any instructions related to the Essure procedure, will be withdrawn from Clinical Policy 1E-3, Sterilization Procedures, as of August 15, 2020.
4. Bilateral salpingectomy as a preventative measure
Prophylactic salpingectomy operations will be covered as a sterilization procedure for North Carolina Medicaid beneficiaries beginning Aug. 15, 2020. According to studies, completely removing the tubes at the time of sterilization may reduce the incidence of ovarian cancer. In Attachment A, Clinical Policy 1E-3, Sterilization Procedures, the following CPT codes will be added.
- 58661 – Surgical laparoscopy with adnexal structures removed (partial or whole oophorectomy and/or salpingectomy)
- 58700 – Salpingectomy, unilateral or bilateral, complete or partial (separate procedure)
In addition, in Attachment B, Clinical Policy 1E-3, Sterilization Procedures, the following associated abbreviations and approved written phrases for use on the sterilization permission form have been added.
Prior to performing a Prophylactic bilateral salpingectomy, providers should review Clinical Policy 1E-3, Sterilization Procedures for general wording and formatting revisions, as well as instructions on billing and completing the sterilization consent form.
