Does Molina Healthcare cover eyeglasses?
Molina Healthcare covers one annual eye exam by a provider, ophthalmologist, or optometrist. Every two years, Molina Healthcare will cover one pair of eyeglasses (frames and lenses).
Replacement eyeglasses are only available to members 21 and older when medically necessary. Replacement eyeglasses are not restricted for members under the age of 20.
Members who choose frames outside of the permitted options will receive a $40 credit toward their eyewear benefit (lenses and frames) once a year from Molina Healthcare.

Who is David Molina?
Conrad and Josephine Molina’s only son, C. DAVID MOLINA, MD, MPH, was born on September 7, 1926. He was born and raised in Yuma, Arizona, before moving to California for education. He met his wife, Mary, at San Diego State College, and they graduated together in 1951. At Yuma Indian School, he taught sixth, seventh, and eighth grades. He later taught at Long Beach’s Mark Twain Elementary School before enrolling in medical school.
Dr. Molina graduated from the California College of Medicine with a medical degree (the precursor to the University of California, Irvine School of Medicine). Following a preceptorship at St. Mary Medical Center in Long Beach, he established a private internal medicine practice. Dr. Molina founded the first Intensive Care Unit in Long Beach in 1962 at Pacific Hospital. Until 1976, he was the director of the ICU. He continued to be interested in critical care and was inducted into the American College of Angiology as a fellow. Soon after, Dr. Molina was named director of Pacific Hospital’s emergency department, a position he held for 21 years. He was also a founding member of the American College of Emergency Medicine’s California chapter. Dr. Molina also secured a funding to build Long Beach’s mobile emergency medical treatment system, and in 1971, he trained the city’s first firefighter paramedic teams.
Dr. Molina enrolled in the Master of Public Health program at UCLA in 1969, and got his MPH with a focus in medical management in 1970, seeing changes on the horizon. Dr. Molina’s initial foray into managed care was as a consultant for General Medical Centers Health Plan in Pomona, where he spent six years working in the areas of HMO formation and administration.
Dr. Molina founded Molina Medical Centers in 1985, and the Department of Health Services awarded him a contract under the new Primary Care Case Management (PCCM) program. With 12 staff-model offices and 10,000 members, Molina Medical Centers became the state’s largest PCCM contractor in under five years. In 1994, Molina Medical Centers earned its Knox-Keene license, making it the first PCCM in ten years to make the switch from partial to full risk. Molina Medical Centers grew to over 105,000 members during his time as president and CEO. Molina’s provider network consisted of 29 staff-model offices and 8,000 more providers spread throughout eight counties. The Mainstream Medi-Cal contracts for Riverside and San Bernardino counties were given to Molina Medical Centers in 1995.
On December 8, 1996, Dr. Molina passed away. He and his wife had been married for over 40 years at the time. In November of 2012, Mary died away. Mario, John, Martha, Janet, and Josephine are their two adult sons and three daughters, as well as their spouses and children.
Dr. Molina was a health-care leader in California for nearly 35 years. His work embodies the advances that have marked developments in health care, and his legacy lives on via his children and the firm he founded, Molina Healthcare, Inc., which is now publicly traded and listed on the FORTUNE 500.
